Make your nomination by completing our online nomination form.
The online nomination form is to be completed by department personnel for the Residency Program Director or Chairman. A link to this form will be emailed to Residency Program Directors and Chairmen before the end of March. If your institution has an awards ceremony, SLS will send the nomination packet in time for you to make your presentation to your Outstanding Resident, or the packet can be sent to the Resident’s address.
